Self-test failed out of box

I see others have asked this question but there are no answers, so I'll ask again.

Just got one for my birthday, opened it up, plugged it in, red LED blinks 5 times. After a couple hours of struggling, I finally got a console to connect to it. The log (couldn't copy/paste, so forgive any typos):
